I created an Outlook e-mail message form. It appears fine on my screen when I
go to File, Chose Form, (select the form I created). I type the info into the
form, hit send. When opened by the receipient it is blank, except for the
subject line.

I created a form and added a From text field on th read page. However, when I
run the form the From field is empty. I've even copy and pasted this field
from other forms where it works and made sure all the seetings are the same
and it still doesn't work.

A form that I created always sets "Have replies sent to:" in the "Message
Options" settings with today's date stamp. This requires to user to either go
in and uncheck this setting or go through several dialogs to delete the date
stamp prior to sending the form.
