I created a custom form to record information (school attendance) and
convert that information to a journal note for a contact.  What I
can't figure out is how/where to put a button or a menu item in
Outlook so I can access the info easily.  the only way I can do it now

I've customized post form in OL2007,published it to a folder (under
personal folders) & now want to make it as default form.So when user

Everything is working great except the form goes away when the people hit
reply. I need the person who receives the e-mail to reply to all and finish

We created a custom form with some coding in it.   We are using Outlook 2003.
 The form when SEND was clicked would go to the person that it was addressed

As we know that when someone receive the meeting request then they will
accept it by clicking accept button or reject or select tentative.  Is it

I do not have this problem at my computer at Outlook 2003 EN SP3, but my
colleague has this problem at the same version at his computer.
The problem looks like there is Form with Combo box with multiple posible
