I have a long list of 'mail merge' in my Tools drop down list.  I don't think
I set it up and don't know how to turn it off.

In word, I have a mergedfield pointing to an Access bd field where is a
number. Now I need to use comas (,) intead of points (.) as my decimal

Does mail merge support using the labels defined within Excel as the Header
field?  I would think that it does, but when I attach to the Excel file it
doesn't automatically map, but instead shows me the first record and I have
to then manually map it, i.e.:

When I hit merge it is not going to the database I have selected.  When I use

I am not sure this really a mailmerge issue, but here goes.
I want to display a form that has a series of options to the user.  In front
of each option there will be a check box.  So the end results looks like:
