For some reason, every time I work with a document that
contains form fields (created using the "Forms" toolbar),
the "Controls" toolbar appears spontaneously and chooses
to go in and out of Design Mode without rhyme or reason.  

How do you insert custom templates in Word 2000 so that
they show up on the listing when you go to File -> New
Document. I used to be able to do it by going through
