Every time I open a specific document I am working on, the floating
Control Toolbar appears. I can close it for a specific session, but the
next time I open the document it's there again. How do I get rid of this
little pest permanently?

My normal.dot file has been corrupted, for example I cannot see in a word
document my logo which is a .jpg file, plus the show/hide icon doesn't do
it's job.  
My question, where and how do I get my normal.dot file back to its original.

These line number are already in a document that I must edit and remove. I
tried suppressing them and unchecking the add line numbers box in page layout.
