I opened a new MS Word blank document only to find the cursor at the top of
the page above the 1" margin.  While typing text, the program appears to
acknowledge the left, right and bottom margins, but not the top.  When I
Print Preview, the document appears to use the 1" margin ...

I was working with a document that had line numbering enabled. I was
using "track changes." I don't know what happened, but all of a sudden
the phrase "CHAPTER 1" appeared before all the line numbers throughout
the whole document, in chapter 1 and otherwise.

For no apparent reason, my document chooses to go to the next page when I
apply a heading style to a line, leaving too much white space on the previous
page.
If I change the style to a non-heading style, it returns to the previous page.

We have a letterhead that part of the header is printed down the left side of
the page, making the left margin 2.5".  On the 2nd page, we want the margin
to go back to 1" on the left.  
Right now we have two templates, for a one page and a two page letter.
