I am using Word 2000 and I continually get the following error message:  "Error! No table of contents entries found."  What on earth am I doing wrong?

I am creating a document that consists of multiple tables
that contain text form fields.  These tables span over
several pages.  The document contains a header and a
footer already.

Is there a way I can insert a chart in Word and have it linked to a table I have in the same document? So that when I change the number in the Word table, the chart will update also
